If the enrolling student is under 18 years of age, legal guardian to acknowledge on behalf. We will reply to your application by email and your enrolment will be confirmed when payment is complete.

I consent to photos of my child being taken and understand these photos may be used for the promotion of F’Aerial Arts Circus School including but no limited to social media usage.
